SLB is an in-home care service provider for medically fragile children throughout the state of Georgia. We serve children with various conditions such as Spina Bifida, Cerebral Palsy, Wheelchair Bound, Muscular Dystrophy, Hydrocephalus and many more. We are an approved GAPP provider (Georgia Pediatric Program) focused on providing quality professional private home care.
We are here to give you and your child the professional support and care that you need to feel comfortable and a little more stress-free. We provide no out of-pocket cost In-home care services to children under the age of 21, who may have significant developmental and/or physical disabilities. Many of our families are surprised to hear that they qualify within 30 days of their application submission.

Getting In Home Care Is As Easy As :
Step : 1
Initial Call and Intake
We gather information as a preliminary assessment. Then schedule an in-home meeting with our client and their family. During the in-home meeting, we further assess the needs of our client and come up with an individualized care plan.
Step : 2
Individualized Care Plan
We create personalized care plans based on client needs and assessments. After thoroughly reviewing the plan with the client and family, we confidently proceed with matching a trusted, qualified home caregiver.
Step : 3
Caregiver Match
With your approval, we efficiently match caregivers and nurses using a specialized algorithm. Detailed care notes are recorded in a logbook, and a dedicated supervisory nurse conducts regular unannounced visits.

GAPP Services
The Georgia Pediatric Program (GAPP) is a Medicaid program that provides services to medically fragile children ages 5-21, who are in need of skilled and unskilled services. There is NO OUT-OF-POCKET COST to the family. Services are approved based on the child’s medical needs, and families who are eligible for Medicaid can apply for GAPP at any time.
